Helse i Hardanger makes arrangements for patients with chronic health conditions to receive highly competent help to formulate tailored treatment goals where they take part in their own health change project at every step.

INTRODUCTION

Health in Hardanger has an interdisciplinary and holistic approach. Within the framework of a concentrated group offer, HiH arranges for patients with chronic health conditions to receive highly competent help in formulating tailored treatment goals where they take part in their own health change project at all stages. INNOVATIVE HEALTH SERVICES The treatment format is interdisciplinary and independent of diagnosis, with a short time in

institution and a long time for preparation and follow-up.


The various interdisciplinary teams include specialists who are relevant to

the underlying disease. The electronic platform that Youwell and HiH have

developed is used throughout the course. TREATMENT PROCESS Preparation

  • Outpatient investigation and mapping of

  • the health problems.

  • Clarification interview, preliminary investigation,

  • introductory evening and telephone included

  • data controller.

Change

  • 4-day stay in Øystese. Interdisciplinary

  • treatment in groups of up to 10 people.

  • A turnaround with a focus on testing

  • scope and tolerance limits.

Maintenance

  • Make the change last.

  • Digital home follow-up where clinicians monitor progress and results.

  • Fixed training at fixed times and places to establish a training routine.


Youwell's roles

Work assignments

Youwell is responsible for adaptation, management and operation of the platform as well as design and development of a web app for the patients/those in need of help. Youwell has also been advisers for the development of the digital interventions.

The project's approach has been service travel design, prototype development, and flexible iterative development and testing phase. In parallel with this, there has been training work in creating good digital interventions and how the patient user experience should be designed.
